As the parent or legal guardian, I release and hold harmless Lucci Spa, its owners, operators and franchisor from any and all liability, claims, demands and causes of action whatsoever, arising out of or related to any loss, damage of injury, including death, that may be sustained by the participant and/or undersigned, while in or upon premises or any premises under the control or supervision of Lucci Spa Kids Spa professionals, its owners and operators or enroute to or from any of said premises
The undersigned gives permission to Lucci Spa, its owners, operators and franchisor to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.
Photographic Material
We take great pleasure in sharing the joyous moments of birthday parties and camp days, as well as the delightful experiences children have at our studio, on our Facebook and Instagram pages. The photos and videos captured during these occasions may be utilized for advertising and promotional purposes on our website and marketing materials.
As the parent or legal guardian of the aforementioned child, I hereby grant Lucci Spa my consent to utilize the photographs taken during this event for any lawful purpose, which includes, but is not limited to, publicity, copyright purposes, illustrations, advertising, and web content.
Moreover, I acknowledge that I shall not receive any form of royalty, fee, or other compensation as a result of such usage.
